About the Role:
Join Skilled Nursing Pharmacy as a Nurse Consultant and make a meaningful impact in the lives of our patients. In this pivotal role, you will collaborate with healthcare teams to enhance patient care and optimize medication management.
Responsibilities:
- Conduct comprehensive medication reviews for skilled nursing facilities.
- Provide clinical consultation to nursing staff and healthcare providers.
- Develop and implement individualized patient care plans.
- Educate staff on best practices for medication administration and safety.
- Monitor patient outcomes and recommend adjustments to therapy.
- Participate in interdisciplinary team meetings to enhance care delivery.
- Stay updated on industry regulations and ensure compliance.
- Assist in training and mentoring new nursing staff on medication protocols.
Requirements:
- Current RN license in Washington State.
- Minimum of 3 years of nursing experience, preferably in skilled nursing or long-term care.
- Strong knowledge of pharmacology and medication management.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a healthcare team.
- Detail-oriented with strong organizational skills.
- Experience in clinical consulting or education is a plus.
- Proficient in electronic health records and documentation.
